General Information on Health Insurance

As with all insurance plans, when you purchase health insurance coverage, you should have been mailed a summary of plan benefits and ID card after your application was approved. The summary of plan benefits lists the benefit and coverage provisions of your insurance plan and the ID card indicates your contract number and your preferred provider (PPO) network.
 

 

 

You are entitled to receive an Explanation of Benefits (EOB) for each claim submitted by you or by your health care provider on your behalf. Not all claims are eligible for payment or for payment in their entirety. The EOB provides an explanation of how a claim has been adjudicated. In other words, the EOB indicates if the claim is an eligible expense under the provisions of your plan and, if so, the level of payment the plan provides.
 
The EOB also indicates if the provider network (PPO) discount has been given and the savings amount applied to your claim.Unfortunately, some claims may not be paid promptly and accurately. If you are having difficulties with your health insurance plan, here are some steps to take:

 

  1. Call the claim administrator identified on your ID card, explain the difficulty and ask for assistance.

  2.  If you have received an EOB and do not understand or disagree with the explanation, inquiries should be submitted in writing to the claim administrator. You may also want to write to the agent, insurance company and provider network with your inquiry.

  3. If you have not received an EOB for a claim having a date of service past 30 days, a written request should be submitted along with a copy of your health care provider’s billing statement.

  4. You are entitled to appeal in writing any claim adjudication under health insurance plans.
