EyeBenefits’ provider network consists of over 9,400 optical store locations nationwide. TheProvider Network includes independent optical stores and major optical chain stores that include Eye Masters, For Eyes Optical, J.C. Penney Optical, Pearle Vision, Sears Optical and Wal* Mart Vision Centers. The vision program also includes discounts on CRT procedures and LASIK surgery with refractive centers in selected markets across the United States.
